Reviews thymosin β4's complex dual role in rheumatoid arthritis (RA). TB4 is expressed in RA synovial pannus tissue where its pro-angiogenic and cell migration-promoting properties may contribute to the pathological tumor-like synovial hyperplasia (foe role). However, TB4's anti-inflammatory activities could limit inflammatory damage (friend role). Reviews evidence for TB4's context-dependent pro- and anti-pathological functions in RA, contributing to the broader literature on TB4's dual roles in angiogenesis-dependent pathological versus regenerative processes.
