Case report of unilateral vitreous hemorrhage in a 55-year-old non-diabetic woman on tirzepatide for weight loss, presenting with sudden vision loss in the right eye. No prior ocular disease was identified. Contributes to the emerging pharmacovigilance literature on incretin therapy-associated ocular adverse events—extending the known GLP-1 RA ocular safety signal (NAION) to a distinct hemorrhagic retinal complication potentially associated with tirzepatide, and raising clinical awareness for ophthalmologists managing patients on GLP-1/GIP dual agonist therapy.
