Case report of semaglutide use for pre-lung transplant weight loss optimization in a 69-year-old man with COPD, T2DM on insulin, and class II obesity exceeding institutional BMI eligibility criteria. Semaglutide enabled sufficient weight loss to meet transplant eligibility thresholds while improving glycemic control. Demonstrates semaglutide's practical utility as a bridge therapy for obese end-stage lung disease patients—enabling access to transplantation for patients who would otherwise be excluded on BMI grounds.
