Case report of a 17-year-old girl with MC4R deficiency (the most common monogenic obesity cause) and severe early-onset obesity with hyperphagia treated with tirzepatide and metformin, documenting effects on hyperphagia severity and BMI. Provides the first reported pediatric case of tirzepatide in MC4R deficiency. Establishes proof-of-concept for tirzepatide in pediatric monogenic obesity with severe hyperphagia—a clinical situation where standard interventions are largely ineffective and where dual GIP/GLP-1 agonism may provide benefit through both central appetite suppression and peripheral metabolic pathways.
