Case report of simultaneous bilateral NAION developing within 14 days of semaglutide initiation (0.5 mg/week) in a 68-year-old man with poorly controlled T2DM and significant hyperopia, progressing to functional blindness within four weeks. Bilateral simultaneous NAION is exceptionally rare. Documents an extreme case of semaglutide-associated optic neuropathy—contributing to the pharmacovigilance literature for NAION and illustrating the potential for rapid, severe bilateral vision loss in patients with structural risk factors (small cup-to-disc ratio, hyperopia) starting semaglutide.
