Case report of the first documented link between oral semaglutide and eosinophilic duodenitis causing high bowel obstruction, with endoscopic confirmation of marked duodenal eosinophilic infiltration. The patient presented with severe gastric stasis and ultimately fecal vomiting after oral semaglutide initiation. Documents a rare but serious eosinophilic GI adverse reaction to semaglutide beyond the typical nausea/vomiting profile—highlighting the need for endoscopic evaluation when GI obstruction symptoms develop during GLP-1 RA therapy.
