Narrative review synthesizing evidence on combining bariatric surgery or endoscopic sleeve gastroplasty with adjunctive anti-obesity medications (including tirzepatide) or probiotics for patients with inadequate weight loss or weight regain after primary bariatric procedures. Reviews evidence for combination efficacy and safety. Provides clinical guidance for the post-bariatric pharmacotherapy combination—positioning tirzepatide as an adjunct to bariatric procedures for patients who experience weight regain or inadequate initial response, where pharmacological augmentation offers an alternative to revisional surgery.
