Case report of unexpected substantial residual gastric contents found at elective upper endoscopy in a patient who withheld one weekly semaglutide dose and followed standard fasting guidelines. The same patient had no gastric residue under identical conditions previously. Illustrates the unpredictability of GLP-1 RA gastric emptying effects even with standard drug holds—reinforcing concerns about aspiration risk variability and supporting more individualized preoperative semaglutide management protocols beyond simple one-week hold recommendations.
