Case report of ischemic colitis developing after six weeks of tirzepatide therapy for weight loss in a previously healthy 32-year-old woman, confirmed by colonoscopy. CT excluded obstruction; stool studies excluded infection; colonic ischemia was the diagnosis. Contributes an important rare serious GI adverse event report to the tirzepatide pharmacovigilance database—distinct from the expected nausea/vomiting profile—raising clinical awareness of ischemic colitis as a potential GLP-1/GIP RA complication requiring prompt evaluation when patients present with hematochezia.
