Case report of a 35-year-old man with class II obesity (BMI 37.8) and severe symptomatic lumbar spinal epidural lipomatosis (SEL) causing neurogenic claudication, who achieved complete SEL resolution with normalization of spinal canal dimensions after tirzepatide-induced weight loss over approximately one year. Provides the first described case of tirzepatide-induced SEL resolution. Extends the GLP-1/GIP agonist SEL treatment literature (previously reported with semaglutide) to tirzepatide—establishing that pharmacological weight loss with dual incretin therapy can reverse severe structural lumbar stenosis from adipose accumulation.
