Case report of a 14-year-old female on semaglutide with autism and developmental delay who had an endoscopy/colonoscopy despite holding semaglutide for 12 days and completing bowel prep—and still had unanticipated residual gastric contents on induction. Highlights that even 12-day drug holds may be insufficient in pediatric patients with developmental comorbidities affecting GI motility. Extends the perioperative semaglutide gastric emptying literature to pediatric patients—a population with unique developmental factors that may further prolong GLP-1 RA-associated gastric emptying effects beyond what adult fasting protocols account for.
