Narrative review examining the emerging roles of mitochondrial-derived microproteins—particularly MOTS-c and humanin—in lung homeostasis and pulmonary disease, identifying gaps in knowledge given that most MDP research has focused on heart, skeletal muscle, and brain. Covers potential roles in pulmonary fibrosis, ARDS, BPD, and lung aging. Highlights lung biology as an underexplored frontier for MOTS-c research—establishing the mechanistic basis for investigating MDPs in pulmonary diseases where mitochondrial dysfunction and oxidative stress are known drivers but MDP-specific evidence remains sparse.
