Case report of a 62-year-old woman without prior cardiovascular history who developed new-onset atrial fibrillation with rapid ventricular response requiring ICU admission shortly after initiating tirzepatide, with no alternative cause identified after workup. Raises the question of tirzepatide's potential role in cardiac rhythm disturbances. Provides pharmacovigilance data for tirzepatide-associated AF—a rare but clinically significant cardiac adverse event requiring prescriber awareness and cardiological assessment of palpitations in new tirzepatide users, particularly as the drug expands to non-diabetic populations without routine cardiac monitoring.
