Retrospective propensity-matched analysis of electronic health records at an academic health center (2010–2021) comparing major adverse cardiovascular events (6-component MACE) in adults with BMI ≥35 and T2DM who received medical weight management (lifestyle ± obesity medications including tirzepatide) versus metabolic bariatric surgery versus usual care. Provides the first MACE comparison including medical weight management as a distinct pharmacotherapy-inclusive arm. Enables evidence-based clinical guidance for prescribers deciding between pharmacological weight management and bariatric surgery for MACE risk reduction in severe obesity with T2DM—informing shared decision-making and insurance coverage decisions.
