Case report of a 40-year-old woman with severe recurrent hypoglycemia unresponsive to conventional treatments (acarbose, glucocorticoids, somatostatin analogues, long-acting GLP-1 RA) in the context of chronic intestinal pseudo-obstruction and esophagojejunostomy, who achieved hypoglycemia resolution with tirzepatide. Proposes impaired incretin function as the underlying mechanism and tirzepatide as a novel solution. Documents tirzepatide's efficacy in refractory post-surgical hypoglycemia—establishing that dual GIP/GLP-1 agonism can resolve hypoglycemia unresponsive to GLP-1-only therapy through a complementary GIP-mediated glucose-stabilizing mechanism in surgically altered anatomy.
