Obese patients showed a significantly blunted growth hormone response to GHRH, and while pyridostigmine partially improved this response, it could not restore it to normal levels. The findings suggest that obesity involves a fundamental disturbance in the cholinergic regulation of GH release, likely due to increased somatostatin tone that cannot be fully overcome by cholinergic stimulation.
