This comprehensive review covers thymosin alpha-1's pharmacology, clinical trials, and therapeutic potential. The synthetic 28-amino-acid polypeptide modulates immunity by augmenting T-cell function, with a serum half-life of approximately 2 hours. Clinical trials showed promise for hepatitis B (40.6% HBV DNA clearance vs. 9.4% controls) and hepatitis C (when combined with interferon alfa-2b: 65% HCV RNA clearance vs. 29% with interferon alone). Additional possible indications include melanoma, hepatocellular carcinoma, drug-resistant tuberculosis, and DiGeorge's syndrome. The drug is well-tolerated with mainly local injection site reactions.
